Alumni Distinguished Undergraduate Professor Award – Submission Packet Guidelines
Submit the paper original and one CD of the entire submission packet outlined below. This includes a high-resolution jpg of the photograph. Each section should be a separate PDF file on the CD. View required CD setup/format.
a. Completed ADUP Nomination form signed by the dean or designee
b. Current CV or resume
c. Brief statement articulating the candidate’s contributions to the teaching mission of the university, not to exceed 2 pages
d. Samples of course materials (such as excerpts from syllabi, assignments, projects, exams, student portfolios, or other evidence of student learning outcome) or documentation of teaching-related activities such as leadership related to teaching and learning or continuing professional development (workshops, institutes, etc.) 4-8 pages
e. Tabulation of student evaluation ratings for each course taught the two years prior to the year of nomination as well as departmental average and range
f. Summaries of two recent peer evaluations (if appropriate for nominee’s appointment)
g. Letters of support from two peers, 1 page each (front)
h. Letters of support from two students, 1 page each (front)
i. 500 word professional bio, suitable for numerous publications
j. One high resolution 5×7 color photograph suitable for publication
- Prepare one paper packet of original submission documents and a copy on a CD of original submission packet.
- Each section should be a separate PDF file on the CD.
- Create a table of contents page for the front of the paper submission packet.
- Print on front and back of pages except for the nomination form. NOTE: Front/back page printing counts as 2 pages.
- Comply with page limits for sections as noted. Exceeding limits will disqualify nomination.
- Use a colored page between sections (a, b, c, etc).
- Binder-clip the paper copy.
Submissions must be delivered to Virginia Howell, 415 Clark Hall, (adjacent to Clark Dining Hall)
